Although U.S. Consular Services in Canada's website is unclear on the issue of HIV (see http://www.consular.canada.usembassy.gov/usa_visa.asp), Form I-690 (below) may not be current, although it was rereferenced at that site as of October 2008 in its instructions document (also below).
